Immunizations for 5th Grade Students

Immunization requirements for school year 2016-2017 for 5th grade students include a meningitis (MCV4) and Tdap shot. Records of these immunizations should be sent in to the school on or before registration in July. They may be faxed to 856-2197 or sent to school with your child. If you need a physician referral, you may contact our school nurses for more information. Immunizations are also available through the Marion County Health Department (317-221-2122) and many pharmacies.
